October 19, 2009: How to enroll via fax, mail or online

When it is time for you to join, we will send you an enrollment packet in the mail.

You can enroll over the phone, online and at community enrollment sessions, and you can also enroll by sending your form by mail or fax.

Fill out the enrollment form that comes in your enrollment packet. Then, you can fax it to us for free at 1-866-482-3676 (1-866-4U2Enroll).
